Application/Agreement

To request funding for your distance education captioning and
transcription needs, please fill out the DECT Application/Agreement
and submit by fax or email. Please fill out
separate applications for each term.
Additional Project Courses

The "Additional Project Courses" page is a
continuation from #3 on the first page of the "Application and
Agreement". Colleges normally have more than one class that
needs captioning services. Please use this page for additional
classes.
Request for
Reimbursement

To request reimbursement for work completed, please fill out the
Request for Reimbursement form and submit by fax or email.
End of Term Report
(MS Word Version)
Please fill out the End of Term Report and submit by fax or email
within six weeks after end of term.
