Affidavit of Financial Support ​ This document is for students with sponsors outside the United States, or who are sponsoring themselves. An affidavit of support is a document an individual signs to accept financial responsibility for another person, usually a relative, who is coming to the United States. The person who signs the affidavit of support becomes the sponsor of the relative (or other individual) coming to live in the United States. ​​
Application​​​​ This is the application document for international students. When you send in the application, make sure to attach a current photo of yourself (aside from your passport or visa picture) as well.​
Approval to Release Student Information​​ This is the approval to release student information document. This document is also found within the ISP application, and is entirely voluntary. If students wish to revoke a previous approval, they may do so at any time by sending an email to isp@canyons.edu with their name, student ID number, and a statement revoking the release.​
I-134, Affidavit of Support​​ This document is for students with sponsors inside the United States. The I-134 form shows that visa applicants have sponsorship and will not become public charges while in the United States. Sponsors must file a separate affidavit for each applicant.​​​​
Mandatory New Student Orientation​
Please submit the mandatory new student orientation sign-up form​​ to the ISP office and RSVP to attend our "Mandatory New Students On-Campus Orientation" meeting. This meeting is specially designed for new international students whose F-1 status is sponsored by College of the Canyons. Every new international student must attend the entire session. Absent and late arrival are not allowed.
Tuberculosis Clearance Please have a licensed physician or medical professional fill out this form. Applicants are required to demonstrate that they are free of tuberculosis infection or on a medically approved treatment plan.
Transfer Clearance​ This document is for ​incoming​ transfer students, and must be completed by the student's transfer-out school. ​
  
Current Student Forms​ ​​
Concurrent Enrollment Agreement​​ F-1 regulations allow an F-1 student to be enrolled in two different SEVIS-approved schools at the same time (concurrent enrollment), as long as the enrollment in both schools amounts to a full-time course of study. An F-1 student whose F-1 status is sponsored by COC should ask for a Reduced Course Load (RCL) authorization to the ISP office by following these procedures:
  1. Make an appointment with the ISP academic counselor.  If your concurrent enrollment is approved by the ISP academic counselor, have this document completed and signed to agree to the conditions authorized to you on the document. 
  2. Make a request for a Concurrent Attendance authorization letter.
  3. Submit a registration record from another school immediately after registration. (3 units = 4.5 units in a quarter system)
  4. Submit a transcript/grade report to the ISP academic counselor to prove that you completed the semester with total of 12 units as soon as that semester is over.
CPT Employment Request​ The regulations state that curricular practical training must be "an integral part of an established curriculum." They define curricular practical training as "alternate work/study, internship, cooperative education, or any other type of required internship or practicum which is offered by sponsoring employers through cooperative agreements with the school. [...]  Student must have been lawfully enrolled on a full-time basis at an DHS-approved school for one full academic year before being eligible for CPT. Available only while student is in F-1 status, before completion of the educational objective."​
The information on this document is collected for the student's reporting purpose to the U.S. Department of Homeland Security during their Curricular Practical Training period. ​​
Document and Transfer Request This document is for students who need to make an I-20 request, a transfer request, a letter request, or any other document request.​​
G-1145, e-Notification of Application/Petition Acceptance Use this form to request an electronic notification (e-Notification) when U.S. Citizenship and Immigration Services accepts your immigration application. This service is available for applications filed at a USCIS Lockbox facility.​​
I-94, Travel Records for U.S. Visitors Use this form to apply for or retrieve your I-94 admission number/record (which is proof of legal visitor status) as well as retrieve a limited travel history of your U.S. arrivals and departures.​
Leave Certificate​ This document is for students who are done studying at College of the Canyons.​
On-Campus Employment: Social Security Number Employer Letter (With COC Logo) This document is for students employed on-campus who are applying for a social security number. This document has the COC logo.
On-Campus Employment: Social Security Number Employer Letter (Without COC Logo) This document is for students employed on-campus who are applying for a social security number. This document does not have the COC logo.
OPT Employment Request​ Student must have been lawfully enrolled on a full-time basis at a DHS-approved school for one full academic year before being eligible for OPT. Available both before and after completion of the educational objective, but different rules apply to pre- and post-completion OPT.
OPT Proof of Employment The information on this document is collected for the student's reporting purpose to the U.S. Department of Homeland Security during their Optional Practical Training period. ​​
Reduced Course Load Request​ Please complete this form and submit it to the ISP front office. An appointment with an academic counselor is required. If requesting Medical RCL, an appointment with your DSO is required first.
This document is to be filled out by a California licensed M.D., D.O., or clinical psychologist only.
SEVIS Student Information: Continuing Student​ Please fill out this document to update your SEVIS information if you have changed your major, added on a second major, changed your contact information, or changed your U.S. address. Check off the box next to the subheadings and fill out ​only the section that has changed.

EFSET Scores​​​​ Here is the interpretation and admittance guide for the EFSET test. Scores are based off of the Common European Framework of Reference for Languages (CEFR) language proficiency​ levels.
F-1 Reinstatement Information​​ A student who has failed to maintain status may be reinstated to lawful F-1 status at the discretion of USCIS, but only under certain conditions.​​
Reduced Course Load Information If an F-1 student is having specified initial academic difficulties, a temporary illness or medical condition, or needs fewer courses than a full course load in their last term to complete the program of study, DSOs may authorize a reduced course load (RCL).
W-8BEN-E, Certificate of Status of Beneficial Owner for United States Tax Withholding​ and Reporting (Entities)​ This form is used by foreign entities to document their status for purposes of chapter 3 and chapter 4, as well as other code provisions.
