Form Information
Once you
completed a registration form, your information will be transferred
onto the Student Development Transcript database. Your transcript
will reflect the co-curricular experiences that you have documented
on the Verification Forms. The Verification Forms are your personal
responsibility. If you do not complete the Verification Forms on a
consistent and timely basis, you will not have a comprehensive
transcript. All Verification Forms must be turned in by the deadline
that is set each semester. Late Verification Forms will not be
accepted. top
