Request for Nursing Prerequisite Evaluation

NOTE: This online form is unique to nursing and no other form can be substituted.

NOTE: You are not required to submit this Request for Nursing Prerequisite Evaluation if:

you have completed a COC Nursing Program Remediation Plan
you have an approved Nursing Prerequisite Evaluation in a previous application cycle

If either apply to you, proceed to the Nursing Program Application Checklist (Step #8)

Prior to submitting this request, you must:

Have completed all prerequisites (Anatomy, Physiology, Microbiology, English 101, Eligibility for Intermediate Algebra)
Apply to College of the Canyons and obtain a student identification number
Submit the following documents to Admissions and Records:
- Official sealed transcripts from all colleges attended, except COC (must include all coursework regardless if nursing prerequisite or not).
- For transcripts from other countries, an official sealed foreign transcript evaluation from a NACES approved agency
- Math placement test results, if needed for documentation of eligibility for Intermediate Algebra
- Official College Board AP English, AP Math, and AP Psychology scores
- Official College Board CLEP Math score
- A copy of high school transcript, copy of diploma, OR copy of GED certificate (if no college degree is posted on transcripts

I previously requested and received a Nursing Prerequisite Evaluation and WAS NOT eligible to apply at that time. I have submitted additional documentation and am requesting a new Nursing Prerequisite Evaluation. Please forward my results to the Allied Health and Public Safety Division Office for the current application period.
