Effective Date of Coverage: Coverage for new employees will begin on the first of the month following the first day of service.

Open Enrollment: There will be an open enrollment period in the fall during which you may change your elections. Changes will be effective October 1st.

Medical Plans

Kaiser (SISC)
HMO. No deductible. Services provided by Kaiser physicians at Kaiser facilities. Prescription drug coverage is included. Eligible dependent children covered to age 26. Eligible dependents who are disabled are not subject to age restrictions.

Blue Cross (SISC)

Dental Plans


United Concordia
Concordia Plus. DHMO (Dental Health Maintenance Organization). No deductible. Services are provided by your primary care dentist, including referral to a specialist. Most procedures are covered at 100%; some require a co-pay. Eligible dependents covered to age 19, 23 if full-time student or disabled. Eligible dependents who are disabled are not subject to age/student restrictions.

Delta
DeltaPremier. No deductible. Services obtained from Delta providers are covered at 70%. This percentage will increase 10% each consecutive year the dentist is visited to a maximum of 100%. Covered prosthodontic services (replacement of missing teeth) are paid at 50%. Payments for services are limited to $2,500 per enrollee per calendar year. Dental accident benefits are covered at 100% with a maximum benefit of $1,000.

Covered services obtained from a non-participating provider are paid at 70% of usual, customary, and reasonable charges. Eligible dependents covered to age 25, regardless of student status. Eligible dependents who are disabled are not subject to age restrictions.

Vision Plan


Vision Service Plan
Coverage includes exam, lenses, and standard frame every 12 months; requires $5.00 co-pay. Contact lenses in lieu of glasses are covered in full if medically necessary, otherwise covered up to $105 if elective. There is a limited benefit for covered services obtained from non-participating providers. Eligible dependents covered to age 19, 25 if full-time student. Eligible dependents who are disabled are not subject to age restrictions.

Group Term Life Insurance


Hartford
Employee only coverage. Plan provides life insurance benefit of $50,000 and accidental death and dismemberment benefit of $50,000. Coverage decreases to $35,000 at age 65, $22,500 at age 70, $15,000 at age 75, and $10,000 at age 80.

Plan also provides a travel assistance benefit. The service provides assistance while traveling, including pre-trip information, emergency medical assistance, and emergency personal services.

Disability Insurance


Hartford
Employee only coverage. Plan provides disability benefit payments equal to 66.66% of monthly salary, to a maximum benefit of $6,000. Benefit is payable after 120 day waiting period.

 

Employee Assistance Program (EAP)


Ability Assist
Program provides employees and their family members with confidential access to counseling and community resources through a 24-hour toll-free hotline. Program provides up to three free counseling sessions per occurrence per year, unlimited referrals to resources, as well as assistance with legal, financial, and work-life concerns.

CalPERS Retirement


California Public Employees’ Retirement System (CalPERS)
Plan provides retirement benefits as well as survivor and disability benefits. Requires employee contribution of 7% of salary, paid through payroll deduction. CalPERS provides members with annual statements.

Group Term Life and Accidental Death and Dismemberment (AD&D)


Hartford
Group Term Life. Employee, spouse, and child coverage. Plan provides group term life insurance benefit up to $500,000 for employee and up to $250,000 for spouse. An additional child benefit in the amount of $1,500, $3,000, or $5,000 is available. Eligible dependent children covered to age 21, 25 if full-time student. Eligible dependents who are disabled are not subject to age restrictions.

Group Accidental Death and Dismemberment (AD&D). Employee or family coverage. Plan provides group AD&D benefit of $10,000 to $500,000, not to exceed ten times your annual earnings.

 

Section 125 Plan


American Fidelity Assurance Company
Plan Year: September 1st through August 31st.
Plan allows participants to pay for qualified expenses on a pre-tax basis. Some examples of eligible expenses are: employee contributions for medical insurance premiums, qualified unreimbursed medical expenses up to $4,800 per plan year, qualified dependent care expenses up to $5,000 per year, and premiums for insurance products offered by American Fidelity such as life and cancer. An election to participate must be made prior to the start of each plan year. 403(b) Plan



This benefit is available to all employees. Contributions and earnings are not subject to current state or federal income taxes. Taxes are deferred until funds are withdrawn. Contributions to the 403(b) plan are not offset by contributions made to the 457 plan. This plan is most commonly used to supplement retirement income.

The basic annual contribution limit is $17,000 for calendar year 2012. Participants age 50 years or older may qualify for a higher contribution limit. Consult with your financial advisor or agent for details. Contributions are made on a tenthly basis, October through July. Participation elections or changes may be made at any time.

457 Plan
