Interpreter Request Form

ASL Interpreter AND/OR Captioning Request Form

NOTE: A separate request must be made for each class/event. This form can be utilized for courses, events, workshops, or any other interpreting/captioning need.

Requests submitted with less than 3 days advance notice may not be filled. If you need to cancel or if you are late, send an email immediately to interpreting@canyons.edu OR call/text Interpreter Services at 805-691-2791. Eligible students may utilize both ASL interpreters AND captioning services for the same course/event. All interpreting or captioning requests must be made via this form. ASL interpreters can accommodate your language needs. If you prefer English based signing, or any other means of signing, please note that below. Captioning can be utilized for both live, in-person classes/events or remote classes/events. If captioning is requested for a live, in-person class on campus, the student can view English captions on their computer, phone, or tablet during the class. Details will be discussed when services are scheduled. If captioning is requested for a remote class, English captions will be within Zoom or other platform. If an instructor asks a student to watch recorded videos or lectures, those too will be captioned. I understand and agree.*
