All CWEE Students
Submission of a Voluntary Activity Particpation Waiver is required for all students seeking to enroll in a CWEE course.
Students will have the opportunity to complete the Voluntary Activity Waiver electronically while viewing the Internship or Work Experience Orientations. Students can also print, complete, scan and return the waiver below to internships@canyons.edu or drop the form off at the Internship Office.
Unpaid Interns Only
Students particpating in an unpaid internship and enrolled in a CWE-186 course are covered for Workers' Compensation Insurance by the Santa Clarita Community College District for the purpose of the internship.
Contact the Internship Office for a copy of the Workers' Compensation: Pre-Designation of Physician form. This form must be completed, scanned and returned to internships@canyons.edu before permission to enroll in the CWE-186 course can be granted.
